Application form and prospectus can be obtained from the office of the Principal on request accompanied by a crossed demand draft for Rs.300/- in favour of Principal, Sri Adi Siva Sadguru Alli Saheb Sivaaryula Ayurvedic Medical College, Guntakal.
The following documents are to be brought at the time of admission with one set of photocopy duly attested by a Gazetted Officer.
